Coast Guard crew crash report delayed further

The Air Accident Investigation Unit (AAIU) has given an update on the fourth anniversary of the R116 helicopter crash, in which four Coast Guard crew members were killed off the coast of Mayo. 

It confirmed that a review into the draft final report is ongoing. The review was ordered after one of the parties mentioned in the draft report raised concerns about some matters in the document.

Lost at sea

Remains of two of the crew, Captain Dara Fitzpatrick and Captain Mark Duffy, were recovered but two others, winch operator Paul Ormsby and winchman Ciarán Smith, remain lost at sea. 

The AAIU must report on the inquiry’s progress if the final report is not published within 12 months of commencement.

The review is being conducted by Patrick McCann SC and is expected to continue for several more weeks.
